Abstract

This Perspective is a conceptual contribution; it does not report an empirical validation study. Lightweight, browser-based clinical calculators are easy to build and distribute, and that ease can be mistaken for validation: correctly reproducing a protocol's arithmetic does not establish clinical safety. Using a research and educational pediatric fluid-calculator prototype that I developed as a worked example, I propose a practical three-layer set of questions for developers and reviewers of such tools: algorithm correctness; guideline fidelity and clinical or model validity; and user safety. I relate these layers to established software, risk-management, usability and clinical-evaluation standards, propose minimum evidence for different categories of tool, and set out a staged pathway with deliverables and exit criteria. A concrete case, fluid-deficit correction in pediatric diabetic ketoacidosis (DKA), shows how a rule can be arithmetically correct yet fail to encode a clinically important conditional distinction in the source guideline. Considerations specific to resource-constrained settings are discussed, including stale offline versions, local protocol adaptation and the absence of post-deployment monitoring. None of the tools discussed should be used for clinical decision-making in its current state.
